You will only have to deal with Customs and Immigration when you enter the Cayman Islands......then, vice versa, on the way home to the U.S., you will need to clear the U.S. Customs and Immigration at the first U.S. airport that your plane stops. In other words, if, on your way home, you are connecting in Houston and going on to somewhere else, you would go through U.S. Customs in Houston. On the way down to Cayman, you wouldn't go through Customs until you land in Cayman, even if you have a connection in Houston.
By the way, when you leave on an international flight from the U.S. they will want to see your passport before they allow you to leave the country (so that you'll be able to get back in ).
